Job summary

Defiance Health Care Center in Defiance, OH is seeking a Licensed Practical Nurse (LPN) to join our compassionate team and provide high-quality resident care. You will perform assessments, administer medications, and coordinate with physicians.

Qualifications

• Licensed Practical Nurse (LPN) licensure is required.
• Ability to conduct assessments and document medical and behavioral changes.
• Experience with medication administration and care coordination is preferred.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
