Job summary

Defiance Health Care Center is hiring a dedicated Licensed Practical Nurse to support residents in a compassionate, resident-focused environment. You will perform assessments, wound care, medication administration, and discharge coordination while supervising CNAs and CMTs to uphold care standards.

Qualifications

Active LPN license in Ohio is required.
Demonstrated patient-care skills with empathy and patience.
Ability to supervise CNAs and CMTs and follow facility protocols.

Responsibilities

Conduct assessments, follow protocol, report changes, and document findings.
Perform admission duties: assessment, MAR, TAR, POS, orders, meds.
Manage discharges: document, notify physician/guardian, arrange transport.
Provide wound care: follow protocols, irrigate, pack, dressings.
Lead CNAs and CMTs to ensure policy compliance.
Respond effectively to emergency situations.
