Defiance Health Care Center is hiring a dedicated Licensed Practical Nurse to support residents in a compassionate, resident-focused environment. You will perform assessments, wound care, medication administration, and discharge coordination while supervising CNAs and CMTs to uphold care standards.

Qualifications

• Active LPN license in Ohio is required.

Responsibilities

• Conduct assessments, follow protocol, report changes, and document findings.
• Perform admission duties: assessment, MAR, TAR, POS, orders, meds.
• Manage discharges: document, notify physician/guardian, arrange transport.
• Provide wound care: follow protocols, irrigate, pack, dressings.
• Lead CNAs and CMTs to ensure policy compliance.
• Respond effectively to emergency situations.

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
