Job summary

Rockville Center is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing facility. The role involves patient care on the SNF/Long Term Care unit, medication administration (including IVs), vital signs, and resident education, with collaboration with physicians, NPs, families, and the care team.

Shifts are 7 AM–3 PM, with every-other-weekend work required. A current NY LPN license is required; benefits include medical, dental, vision and other fringe benefits.

Qualifications

• Current LPN license in NY.
• Willingness to work alternating weekends.

Responsibilities

• Provide resident care in SNF and Long Term Care units with monitoring of outcomes.
• Administer medications, including IVs where applicable.
• Take and record vital signs.
• Deliver resident education and care instructions.
• Communicate with physicians, NPs, families and care team.
• Supervise CNAs on shift.

The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
