Qualifications

• Valid LPN license in PA.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, vital signs, and responses to treatments.
• Assist in development and implementation of individualized care plans.
• Provide nursing care including wound and catheter care and assistance with daily living activities.
• Collaborate with physicians, RNs and other healthcare team members for coordinated care.
• Educate residents and families about health management and procedures.
• Ensure care complies with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising CNAs.

Qualifications for Licensed Practical Nurses (LPN):

• Valid Licensed Practical Nurse (LPN) license in PA

• Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or healthcare setting for at least 6 months.

• CPR Certification

Pre-Hire Requirements

• Current physical examination completed within the past 2 years

• Completed PPD/TB test
