Aventura at Humility House Skilled Nursing Facility in Austintown, OH is seeking Licensed Practical Nurses (LPNs) to provide direct nursing care to residents and work with a collaborative team in a supportive environment.

We offer 12-hour shifts (days and nights), full-time, part-time, and PRN options, with a competitive hourly rate of $25–$29.50 based on experience and shift, plus benefits including 401K, daily pay, health/dental/vision, PTO, and growth opportunities.

Qualifications

• Licensed Practical Nurse (LPN) license in OH.
• At least 6 months of experience as an LPN in a nursing home or long-term care (LTC) setting is required.
• Compassionate, patient-focused, and committed to providing high-quality care.
• Ability to work independently and collaboratively within a team.
• Excellent communication and interpersonal skills.
• Ability to maintain accurate records and handle sensitive information confidentially.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, including vital signs, symptoms, and responses to treatments.
• Assist in the development and implementation of individualized care plans.
• Provide nursing care for residents, including wound care, catheter care, and assistance with daily living activities.
• Collaborate with physicians, RNs, and other healthcare team members to ensure coordinated and comprehensive care.
• Educate residents and their families about health management and procedures.
• Ensure that all care is delivered in accordance with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising nursing assistants (CNA).
