Job summary

Barclays Rehabilitation and Health Care Center is actively hiring Licensed Practical Nurses (LPNs) for part-time, per diem, and weekend shifts. We offer same day pay and a supportive, welcoming team in a senior care setting.

As an LPN you will assist with ADL care, administer medications, monitor patient status, and collaborate with RNs and physicians to ensure quality care. New graduates are welcome to apply.

Qualifications

• Must hold a valid LPN license and be in good standing.
• Experience in long-term care or skilled nursing settings preferred.
• Ability to perform ADL and routine resident care with compassion.

Responsibilities

• Assist CNAs with ADL and routine care.
• Conduct resident/patient treatments under supervision.
• Administer medications per physician orders and Plan of Care.
• Submit pharmacy orders and accurately document all information.
• Administer injections as prescribed and monitor patient responses.
• Observe health status and report changes to RNs/physicians.
• Take vital signs (BP, temp, weight) and monitor trends.
• Perform basic wound care and bandaging.
• Manage IVs and IV drips as directed.
• Track intake and output and report concerns.
• Assist in admissions process as directed.

Key ResponsibilitiesJob responsibilities for Licensed Practical Nurses (LPNs) include and are not limited to:

• Assisting CNAs in performing ADL and routine care.
• Conducting resident/patient treatments.
• Administering medication in accordance with physician orders and Plan of Care.
• Submitting pharmacy orders.
• Accurately documenting and recording all information.
• Giving injections of medication as prescribed.
• Observing resident health to communicate current condition to RNs, Supervisors, and Physicians.
• Taking vital signs, including blood pressure, temperature and weight.
• Basiccare, including wound cleaning and bandaging.
• Managing IVs, starting IV drips.
• Monitoring fluid/food intake and output.
• Observing and reporting resident accidents, incidents and the presence of skin breakdowns.
• Assisting as directed in the admissions process.
