Job summary

Berkshire Post Acute in Wayne, NJ is immediately hiring Licensed Practical Nurses (LPNs) to join our compassionate caregiving team. We offer up to a 1,500 sign-on bonus and a comprehensive benefits package including PTO, holidays, medical, dental and vision coverage.

You will perform direct patient care, assist with activities of daily living, administer medications, monitor vitals, document care accurately, and collaborate with RNs and physicians.

Qualifications

• Active LPN license in good standing.
• Experience in long-term care is a plus.
• Ability to follow physician orders and care plans.
• Strong documentation skills.

Responsibilities

• Assist CNAs with ADLs and routine care.
• Administer medications per physician orders.
• Take and monitor vital signs.
• Document accurately and maintain patient records.
• Submit pharmacy orders.
• Observe and report changes to RNs/Physicians.
• Support admissions processes.

Key Responsibilities

• Assisting CNAs in performing ADL and routine care.
• Conducting resident/patient treatments.
• Administering medication in accordance with physician orders and Plan of Care.
• Submitting pharmacy orders.
• Accurately documenting and recording all information.
• Giving injections of medication as prescribed.
• Observing resident health to communicate current condition to RNs, Supervisors, and Physicians.
• Taking vital signs, including blood pressure, temperature and weight.
• Basic care, including wound cleaning and bandaging.
• Managing IVs, starting IV drips.
• Monitoring fluid/food intake and output.
• Observing and reporting resident accidents, incidents and the presence of skin breakdowns.
• Assisting as directed in the admissions process.
