Jamestown Place Health and Rehab in Jamestown, OH seeks a licensed practical nurse (LPN) to provide direct patient care and supervise CNAs per regulations and facility policies. The role ensures safe medication administration, wound care, admissions and transfers, and ongoing resident assessment.

Ideal candidates hold an accredited nursing degree, an unrestricted LPN license, CPR certification, and IV certification if applicable; prior long‑term care experience is preferred but not required.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• IV certification, if applicable.

Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Promotes holistic care for residents using the nursing model.
• Ensures policy and procedure compliance by nursing staff.
• Participates in admissions, discharges and transfers of residents.
• Assesses changes in residents’ status and notifies physician and family, documenting accordingly.
• Transcribes physician orders to medical records and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects.
• Performs wound treatments as ordered and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to the unit.
