Centerville Health and Rehab in Centerville, OH seeks a Licensed Practical Nurse (LPN) to provide direct nursing care and supervise certified nursing assistants in daily activities compliant with federal, state, and local regulations.

The role involves admitting, discharging and transferring residents, transcribing physician orders, administering medications, wound care, and ensuring high-quality holistic care. Current LPN license and CPR are required, with IV certification a plus.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• IV certification, if applicable.

Responsabilités

• Directs the daily activities of certified nursing assistants in accordance with regulations and facility policies.
• Follow the nursing model to promote holistic care for residents.
• Ensure compliance with policies and procedures by nursing staff.
• Participate in admissions, discharges and transfers of residents.
• Assess changes in residents’ status and notify physician and family, documenting accordingly.
• Transcribe physician orders to medical records and carry out orders as written.
• Prepare and administer medications as per physicians’ orders and monitor for adverse effects.
• Perform wound treatments as ordered and document changes.
• Schedule follow-up appointments and transportation as indicated.
• Provide nursing leadership to the nursing unit staff.
• Perform other duties as assigned.
