Edna Tina Wilson Living Center of Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) for its Long Term Care unit on the day shift. You will provide professional patient care to long-term and post-acute residents and supervise CNAs while ensuring accurate documentation in the medical record.

The role requires NY LPN licensure and BLS, with opportunities for leadership, tuition support, and shift differentials.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association.

Responsibilities

• Provide timely, accurate medication administration and document accordingly.
• Perform treatments and document progress per physician orders and protocol.
• Ensure complete resident care documentation in the electronic medical record.
• Direct CNA team to complete care per individual care plans.
• Transcribe orders, contact physicians, and follow up to institute orders promptly.
• Monitor residents and report status to charge nurse or physician as needed.
• Complete MDS documentation and related observations timely.
• Assist with direct ADL care and support CNA team as required.
