Job summary

Rochester Regional Health seeks a Licensed Practical Nurse (LPN) for Edna Tina Wilson Living Center in Rochester, NY. This part-time evening position supports long-term and post-acute residents and leads a CNA team per care plans.

You will document care, administer meds, and collaborate with physicians and staff to ensure high-quality patient outcomes while advancing in a CMS 5-Star facility with shift differentials and tuition assistance. Evening shift (3:00pm–11:00pm) and every other weekend.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in New York State.
• BLS Certification from the American Heart Association (AHA).

Responsibilities

• Timely, accurate medication administration and documentation.
• Performs treatments and documents progress per orders and facility protocol.
• Maintains concise, complete resident care documentation using the electronic medical record system.
• Directs CNAs to complete care per individual care plans with accurate documentation.
• Receives/gives relevant reports with staff as per protocols.
• Transcribes orders and follows up to institute orders promptly.
• Monitors resident status and communicates to charge nurse/physician.
• Completes MDS documentation and related observations on time.
• Supports CNA team with direct ADL care.
