Job summary

Signature HealthCARE of Roanoke Rapids is seeking a dedicated LPN/LVN to provide direct nursing care, manage medications, and supervise nursing staff in a 108-bed facility. You will support residents with ADLs, document care, and participate in care planning with the interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) with dignity and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ status and report changes to physician and family.
• Develop and review individualized care plans with the interdisciplinary team.
