Job summary

Montefiore is seeking a Licensed Practical Nurse (LPN) to deliver compassionate, science-driven care in a collaborative team setting. You will work under RN/physician supervision to administer medications, monitor vitals, provide wound care, and triage patients.

Qualifications

• Active NY LPN license and registration.
• BLS certification required; infection control training preferred.
• High School diploma or GED required.

Responsibilities

• Direct patient care under RN/physician supervision, including medications, vitals, wound care, prompt triage, and treatments.
• Accurate documentation of patient information, vitals, interventions, and meds with high accuracy; collect and label specimens.
• Maintain exam rooms, stock supplies, and sterilize equipment per protocol.
• Communicate with the healthcare team to escalate issues, facilitate discharge, and educate patients and families.
• Provide direct care for a specific patient population, administer meds and treatments, and offer discharge instructions.

Responsibilities

• Direct Patient Care: Provide hands-on care under RN/physician supervision, including administering medications, monitoring vitals, wound care, triaging/rooming patients within 15 minutes, and performing treatments.
• Documentation & Specimens: Accurately document patient info, vitals, interventions, and medication administration with 100% compliance. Collect, label, and log specimens (blood, biopsy) with zero tolerance for error.
• Coordination & Maintenance: Clean and stock exam rooms between sessions, maintain supply par levels, and sterilize equipment via autoclave.
• Communication & Education: Partner with the healthcare team to **escalate issues**, facilitate discharge, and teach patients/families regarding care management.
• Provides direct care for a specific patient population. Administers medications and treatments. Provides patient & family teaching and discharge instructions.

Requirements

Shift: Monday to Friday 1000A – 0630P with Alternating Saturdays
