Job summary

Montefiore is seeking a Licensed Practical Nurse to deliver nursing care for pediatric, adolescent, adult and elderly patients in ambulatory settings. The LPN will pre-screen patients, educate on telemedicine where applicable, assist in scheduling and outreach, and work under supervision of a RN or MD.

Qualifications

• Must hold NYS LPN license and be registered as an LPN in NY.

• One year of related work experience.
• Basic Life Support certification.

Overview

For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.

The Licensed Practical Nurse is responsible for the delivery of care to the pediatrics, adolescent, adult and/or elderly patients within their scope of practice. He/She participates in performance improvement, research activities and maintains clinical expertise. The LPN will pre-screen patients prior to their visits, educating patients on telemedicine platform where applicable, assist in scheduling and patient screening and assist in chronic disease and preventative care patient outreach.

As a member of the nursing team and under the supervision of a Professional Registered Nurse or Doctor of Medicine, will provide nursing care to patients/clients within assigned practices. Under the direction of a Registered Professional Nurse or Doctor of Medicine, the Licensed Practical Nurse performs tasks and responsibilities within the framework of case finding, health teaching, health counseling and provision of restorative and supportive care, data collection, monitoring and observation, implementation of treatments, and therapies provided to Ambulatory Care patients throughout the age continuum (infants/geriatrics) that fall within the scope of practice.

Responsibilities

• Provide patient/family teaching and discharge instructions to meet learning/ discharge needs as identified by RN.
• Able to set priorities, problem-solve, and use proper judgment in difficult situations and be flexible in non-emergent situations.
• Provides and maintains safe environment for patients/families through preventive and interventional measures.
• Documents direct patient care as per MMC policies and procedures.
• Participates, practices and promotes continuous quality improvement.
• Communicates effectively with patients/families and members of the health care team.
• Responsible and accountable for own practice.
• Other duties as assigned.
  • Completes duties as assigned.
• Sterilize instruments and equipment using the autoclave.
  • Accuracy in sterilization and documentation as per MMC policy.
• Order and maintain medications and supplies to par level for site/s. Ensure exam rooms are appropriately, cleaned and stocked each session between patients.
  • Practice is properly stocked and exam rooms remain clean and stocked with supplies every session and between patients as required.
• Collect and properly label specimens as per MMC policy. Maintain specimen (blood, biopsy) logbooks and report abnormal results to MD.
  • Accurate labeling and documentation of specimens required. Zero tolerance for error.
• Greet and direct patients to exam room.
  • Complete vitals and all other clinical duties directed from provider or RN. Patients triaged and roomed within 10 to 15 minutes of arrival to practice.

Requirements
