Montefiore is seeking a Licensed Practical Nurse (LPN) to join their team in New York, NY. As an LPN, you will provide essential direct patient care and work closely with healthcare professionals to ensure quality health services. Candidates must possess a valid NY LPN License and certifications in BLS and Infection Control.

Responsibilities

• Direct Patient Care: Provide hands-on care under RN/physician supervision, including administering medications, monitoring vitals, wound care, triaging/rooming patients within 15 minutes, and performing treatments.
• Documentation & Specimens: Accurately document patient info, vitals, interventions, and medication administration with 100% compliance. Collect, label, and log specimens (blood, biopsy) with zero tolerance for error.
• Coordination & Maintenance: Clean and stock exam rooms between sessions, maintain supply par levels, and sterilize equipment via autoclave.
• Communication & Education: Partner with the healthcare team to elevate issues, facilitate discharge, and teach patients/families regarding care management.
• Provides direct care for a specific patient population. Administers medications and treatments. Provides patient & family teaching and discharge instructions.
