Job summary

sevencounties is seeking a Licensed Practical Nurse to provide frontline clinical support in Louisville, KY. You will conduct health screenings, triage needs, and help clients access services, housing and community resources, while advocating for timely care.

KY LPN license and at least 3 years of nursing experience in hospital, physician office, or behavioral health are required; community-based care experience is preferred. This full-time, on-site role requires reliable transportation.

Qualifications

• Active KY LPN license is required.
• Minimum 3 years of nursing experience in hospital, physician office, or behavioral care clinic.

Responsibilities

• Provides first point of clinic contact for new clients and explains services.
• Completes health screenings, nursing assessments and triage healthcare needs.
• Participates in team meetings to develop a care plan.
• Assists clients in navigating the healthcare environment for access to care.
• Advocates for clients with providers and managed care organizations to ensure timely care.
• Coordinates access to housing and social support as needed.
• Identifies other needs that affect healthcare access and outcomes.
• Documents appointments, medications and needs for assigned clients.
