Qualifications

• Licensed Practical Nurse with current active license in Kentucky.
• Three years of nursing experience in hospital, physician office, or behavioral care clinic.

Responsibilities

• Provides first point of clinic contact for all new clients.
• Explains services available and engages clients to access services.
• Completes health screenings, nursing assessments and triage healthcare needs.
• Assists clients in navigating the healthcare environment to access appropriate care.
• Advocates on client’s behalf with healthcare providers and managed care organizations.
• Coordinates access to services addressing social determinants of healthcare outcomes.
• Provides transportation to healthcare appointments when needed.
• Develops knowledge of other healthcare providers in the area and collaborates as needed.
• Educates and supports wellness activities such as healthy eating and physical activity.
• Assists with transitions from inpatient or residential care to outpatient care to prevent readmissions.
• Collaborates with client and family to develop support networks.
