Job summary

NHC HomeCare Lewisburg is seeking an Intake/Scheduler, LPN to support the coordination of home health services. This role works closely with our clinical and administrative teams to help keep patient care organized, communicate important updates, and support efficient scheduling.

The ideal candidate has a high school diploma or GED, at least 2 years of home care experience, and strong communication and organizational skills.

Qualifications

• High school diploma or GED required.
• 2+ years of experience in home care or related healthcare setting.
• Computer experience required.
• Current LPN license preferred in the agency's state.
• Valid driver’s license and reliable transportation.
• Strong customer service and telephone skills.
• Excellent communication and organizational skills.

Responsibilities

• Coordinate scheduling and approved visits for patients receiving home health services.
• Assist with obtaining approvals for additional discipline visits for managed care patients.
• Receive and route physician orders to the appropriate homecare team member.
• Communicate order changes to patients and caregivers.
• Review physician orders with the DON or RN designee before ordered care is provided.
• Facilitate communication of laboratory results to the appropriate team member.
• Assist with timely communication between the homecare team and physicians.
• Document communication related to patient care coordination.
