Job summary

BayCare HomeCare in the Tampa Bay area is seeking a compassionate Home Health Field LPN to provide hands-on nursing care in patients’ homes.

You will deliver patient care under the supervision of a Registered Nurse, develop teaching plans for patients and caregivers, and follow the care plan while coordinating with RN for changes in patient status. A valid Florida LPN license and BLS are required, with at least 1 year of home care experience.

Qualifications

• BLS certification required.
• LPN license and valid Florida nursing credential.
• Associate's or Diploma in Nursing required.
• Minimum 1 year LPN experience and 1 year Home Care experience.

Responsibilities

• Deliver patient care in the home, including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse.
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse.
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status.
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

• Home visits in assigned service area: Pasco County surrounding areas

The Home Health Field LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health Field LPN - Requirements:

• BLS (Basic Life Support)

• LPN (Registered Nurse) License

• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

• Required Associate's or Diploma Nursing

• Required 1 year Licensed Practical Nurse

• Required 1 year Home Care
