Job summary

RHA Health Services, LLC is hiring an IDD Facility Nurse Licensed Practical Nurse (LPN) in Walnut Creek, CA. Regular position with pay of $33.00 per hour and a $6,000 sign-on bonus. Provide holistic nursing care, support the RN, and train unlicensed staff to deliver quality care.

Requirements include an accredited LPN program, an active license, CPR certification, strong communication, and ability to lift up to 40 pounds. Comprehensive benefits and ongoing training are offered.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current nursing license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to obtain certification.
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
• Must be able to lift a minimum of 40 pounds.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities
• Assist the Registered Nurse in delivering quality nursing services to persons supported
• Provide health related training to unlicensed personnel
• Administer medications and treatments or assist the person supported with medication administration
• Assist with ensuring preventative and routine medical appointments are scheduled
• Conduct medication closest assessments
• Collect health related data
• Observe for and report any abnormal health related findings to the physician
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
