Job summary

Kaiser Permanente nurses are guided by an integrated nursing model that places patients and families in the center. The LPN I provides direct and indirect nursing care under supervision to promote health, educate patients, and coordinate care with the Health Care Team.

Responsibilities include preparing patients for visits, completing POE, assisting with procedures, documenting care, educating patients, scheduling tests, and participating in quality improvement and care coordination.

Qualifications

• Graduate of an accredited nursing (LPN/LVN) program.
• Practical Nurse License (Georgia) required at hire.
• Basic Life Support required at hire.
• Complete a Customer Service Assessment.
• Minimum six (6) months of Health care experience preferred.
• Customer service aptitude demonstrated through Customer Service Assessment.

Responsibilities

• Prepares patient for office visit by reviewing history, collecting clinical information, measuring vital signs, and documenting.
• Completes POE and addresses care gaps during the patient encounter.
• Prepares room and equipment for procedures and assists the practitioner as needed.
• Initiates interventions and documents responses according to guidelines.
• Participates in emergency care as directed.
• Educates patients and provides preparation materials for diagnostic studies.
• Schedules appointments for labs, radiology, or specialty consultations.
• Monitors electronic messaging and emails related to patient or provider requests.
• Participates in quality improvement activities and engages in care coordination.
• Resolves patient concerns promptly.
• Provides departmental orientation and training of new team members.

Education

Graduate of an accredited nursing (LPN/LVN) program.
