Job summary

Kaiser Permanente is seeking a Licensed Practical Nurse (LPN) Navigator to coordinate care with physicians, nurses, and staff across departments. The role emphasizes efficient scheduling, referral management, and redirection of urgent needs within the Kaiser network.

The LPN Navigator will review data prior to visits, utilize standing orders and guidelines, and independently manage multiple scheduling tasks to align with treatment plans and patient care goals.

Qualifications

• Minimum three (3) years of current clinical nursing experience.
• Minimum two (2) years of relevant specialty experience.
• Practical Nurse License (Georgia) required at hire.
• Basic Life Support required at hire.
• Experience working in an outpatient medical office preferred.

Responsibilities

• Coordinate direct template management in daily coordination of member appointments and ensure scheduling with the appropriate subspecialist.
• Serve as a resource for internal and external providers with urgent referrals.
• Ask pertinent questions to ascertain the need for an office visit and assist in scheduling based on department guidelines.
• Facilitate member appointments triaged by the primary physician or practitioners for follow-up encounters.
• Maintain accurate medical records and disability forms; communicate with external providers and patients to obtain necessary records.
