Job summary

Billet Health is seeking Licensed Practical Nurses (LPNs) for part-time/per diem Home Health roles. You will provide skilled nursing care in patients' homes or residential facilities, working with an RN Case Manager and Director of Nursing to assess needs and implement plans of care across a 9-week episode of care.

You can set your own pace with flexible scheduling, travel within your established territory, and competitive hourly pay.

Qualifications

• Active LPN license in the state of practice
• At least 1 year of Home Health experience
• Valid Driver's License
• Valid Auto Insurance
• CPR or BLS Certification
• AZ Level One Fingerprint Clearance Card (Arizona applicants only)
• Physical/medical clearance to perform job duties (Nevada applicants only)
• Valid TB Test — skin test within the last year or chest X‑ray within the last five years

• Assess patient needs and communicate changes in condition to the RN Case Manager and Director of Nursing
• Follow established plans of care and provide appropriate nursing interventions
• Monitor and document patient progress and care provided

• Travel to patient locations throughout your assigned service area

Billet Health | Part-Time / Per Diem | $45+ per hour | East Valley & Surrounding Areas

Billet Health is looking for Licensed Practical Nurses (LPNs) who bring both clinical expertise and a genuine heart for patient care. Our Home Health team provides compassionate, individualized care to geriatric patients in the comfort of their homes or residential facilities throughout our service areas.

As an LPN with Billet Health, you will work alongside an RN Case Manager and Director of Nursing to assess patient needs, provide quality care, and support each patient throughout their 9-week episode of care.

• Flexible scheduling – You coordinate visit times directly with your patients.
• Choose your workload – Take on as many or as few visits as fit your schedule.
• No assigned visit times – You arrange visits based on what works best for you and your patients.
• Typical visits are 45–60 minutes.
• Travel included within your established territory.
• Additional compensation may be available for patients outside your normal territory on a case‑by‑case basis.
• Work independently while having the support of our clinical and staffing teams.
• Gain experience in Home Health with support from an experienced team.

• West Valley and surrounding areas, including:
• Mesa
• San Tan Valley
• Queen Creek
