Job summary

Emblem Home Health in Arizona is looking for a Licensed Practical Nurse (LPN) to provide hands-on patient care in home health settings under RN supervision. This full-time role requires travel across the West/Central area, a valid AZ LPN license, CPR certification, and reliable transportation.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the State of Arizona.
• CPR certification and valid driver’s license with reliable transportation.
• Preference for 3 years of nursing experience (Home Health or Med/Surg).

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high-quality service.
• Participate in professional development and team meetings.
