Job summary

Billet Health is seeking compassionate Home Health Licensed Practical Nurses (LPNs) to join our team in the Phoenix Valley, Arizona. Per diem or full-time positions offer flexible scheduling as patients come onto service. You will work with the RN Case Manager and Director of Nursing to assess needs, provide skilled care, and ensure timely documentation.

Typical visits range 45–60 minutes. Responsibilities include assessing patients, following care plans, and educating patients and caregivers

Qualifications

• CPR or BLS certification is required.
• Active LPN license in Arizona is required.
• Valid driver’s license and reliable transportation are required.
• Current auto insurance is required.
• TB documentation is required (TB skin test or chest X-ray).

Responsibilities

• Provide skilled nursing care in patients' homes or residential facilities.
• Assess patient needs and report changes in condition.
• Follow the established plan of care and document progress.
• Coordinate with RN Case Manager and Director of Nursing.
• Educate patients and caregivers on care plans and procedures.
• Promote patient safety, comfort, and independence.
