Job summary

Wentworth Hospice, An Amedisys Company, is seeking a licensed practical nurse to join our home-based hospice team in Somersworth, NH. You will verify plans of care with the RN, monitor patient conditions, and educate families to support care at home.

We offer competitive pay of $33.00–$38.00 per hour, choose from PPO or HSA medical plans with telehealth, PTO, and a 401(k) with company match. Travel is required, and driving is supported by reliable transportation and insurance.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when conditions change.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
