Qualifications

• Arizona Licensed Practical Nurse license required.
• 3 years nursing experience preferred (Home Health or MedSurg a plus).
• Current CPR certification required.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high-quality service.
• Participate in professional development and team meetings.
