Job summary

Home Sweet Home Health Agency in Central and South Phoenix is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients’ homes under RN supervision. You’ll follow individualized care plans, document visits in EHR, and communicate with the care team to ensure high-quality home health services.

The role emphasizes flexible scheduling, patient-centered care, and collaboration with a supportive nursing team focused on improving independence and safety at home.

Qualifications

• Current Arizona LPN license in good standing.
• Graduate of an accredited vocational nursing program.
• Minimum 1 year of nursing experience (home health experience preferred).
• Physical ability to assist and move patients as needed.
• Strong communication and documentation skills.
• Proficiency with electronic charting and care coordination software.
• Valid driver’s license, reliable transportation, and ability to travel to patient homes in Central Phoenix and surrounding Maricopa areas.

Responsibilities

• Provide skilled, patient-centered nursing care based on individualized care plans.
• Accurately document all visits, assessments, and treatments using electronic health records (EHR).
• Communicate effectively with patients, families, and the home health care team.
