Job summary

UnitedHealth Group's Northern Arizona Home Health invites a Licensed Practical Nurse (LPN) to deliver high-quality patient care in patients' homes across Parks, AZ. This role supports care coordination, documents visits in EMR, and adheres to infection control and regulatory requirements.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reports changes in condition, and communicates with patient, family, physician, and team.
• Function across home settings regardless of age, race, creed, color, sex, disability, or financial condition.
