Job summary

Amedisys is seeking a licensed practical nurse to support patients at home in a full-time day shift role. You’ll verify plans of care with the RN, collaborate with physicians, teach self-care, monitor vitals, and ensure safe, compassionate care.

The position offers attractive pay ($35.00–$40.00 per hour), benefits including medical plans, PTO, HSAs, wellness rewards, and a 401(k) match. Valid driver’s license and CPR certification are required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
