Job summary

Interim HealthCare Home Health is seeking a Licensed Practical Nurse (LPN) for home visit care. You will provide in-home direct patient care, perform skilled assessments, and develop comprehensive care plans tailored to individual needs.

Clinicians ensure accurate documentation, collaborate with other providers, and educate patients and families. A focus on reducing readmissions and delivering high-quality, patient-centered care is essential.

Qualifications

• Graduate of a practical (vocational) nursing program for LPN/LVN.
• Current license to practice as LPN/LVN in the applicable state.
• At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Complete skilled assessments and deliver patient-centered care in the home.
• Ensure evaluation and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to deliver care.
• Complete all clinical documentation per agency protocol and Medicare guidelines.
• Provide education to patients and families involved in care.
• Work to decrease readmissions by promoting preventative care and continuity of care.

As ALicensed Practical Nurse (LPN) Visit Nurseyou Will

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Complete skilled assessments and provide exceptional patient-centered care in the home
• Ensure an accurate evaluation and implement a comprehensive care plan that addresses patient needs
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve
• Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Provide education to patients and family members involved in patient’s care.
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care

To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state(s) in which he/she practices and have at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Two (2) years of experience practicing as a Licensed Practical Nurse (LPN) in a home health or similar setting
