Job summary

Interim HealthCare is seeking a Licensed Practical Nurse (LPN) to visit patients in their homes, providing direct patient care per the plan of care and ensuring continuity of care. The role emphasizes compassionate, patient-centered home health services and collaboration with the care team.

The position requires LPN licensure, CPR/BLS certification, reliable transportation, and at least 2 years of home health experience. Flexible FT/PT/PRN schedules are offered with competitive pay and benefits.

Qualifications

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license
• Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state of practice
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Two (2) years of experience practicing as an LPN in home health or similar setting

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care
• Complete skilled assessments and provide patient-centered care in the home
• Ensure an accurate evaluation and implement a comprehensive care plan
• Communicate patient conditions and collaborate with providers to deliver care
• Complete clinical documentation in accordance with agency protocol and Medicare guidelines
• Provide education to patients and family members involved in patient’s care
• Work to decrease readmissions by promoting preventative care and continuity of care

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Complete skilled assessments and provide exceptional patient-centered care in the home

To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state(s) in which he/she practices and have at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
