Job summary

Interim HealthCare is seeking a Licensed Practical Nurse (LPN) for in-home patient care on the 11pm-7am shift. You will provide hands-on nursing care, administer medications, monitor patients, and document progress to support comfort and dignity for clients.

Ideal candidates hold an LPN license, CPR/BLS/First Aid certification, and reliable transportation. In-home visits offer one-on-one patient care with daily pay options and a stable schedule.

Qualifications

• Graduate of a practical nursing program.
• Current LPN/LVN license in the state of practice.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Assess and document patient conditions, vitals, and changes as needed.
• Administer medications, insulin, and IV/fluids as prescribed.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Collaborate with providers and staff to ensure optimal patient outcomes.

As aLicensed Practical Nurses (LPN)you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV/fluids,documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:

• Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the OCan contact ffice's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trachand/or ventilator experience preferred, not required
