Job summary

Elara Caring is seeking a Licensed Practical Nurse for a Home Health PRN role in Owosso, MI. The position supports patient-centered care and relies on coordination with an RN Case Manager to update care plans.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Experience in a hospice or home health environment is preferred

• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
