Job summary

Elara Caring in Winnebago, IL is seeking a licensed practical nurse (LPN) to provide in-home health services across Rockford and surrounding areas. You will follow individualized care plans, document care, and collaborate with the RN Case Manager.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Must possess a valid driver''s license or have reliable transportation to travel between patient homes and locations.

Responsibilities

• Follow each patient''s individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive patient care.
