Job summary

CenterWell Home Health is seeking a Patient Services Coordinator-LPN to schedule patient visits, coordinate with field staff and physicians, and support daily office operations. The role includes maintaining logs, managing add-ons and reassigned visits, and ensuring timely communication across teams.

The ideal candidate holds an active nursing license in the state, has at least 1 year of home health experience, and demonstrates excellent communication and organizational skills.

Qualifications

• Must be a licensed nurse in the state of practice.
• Minimum 1 year of home health experience.
• Coding certification is preferred.
• Valid state driver’s license and automobile liability insurance.
• Excellent communication and strong organizational skills.

Responsibilities

• Schedule visits for patients and manage staff calendars.
• Coordinate communication among patients, field staff, and physicians.
• Maintain hospitalization and coordination logs; send required reports.
• Assist with on-call rotations and back-up coverage during busy times.
• Process lab reports and route to the Case Manager and Medical Records.
