Job summary
Tufts Medicine Care at Home in Lowell, MA seeks a Licensed Practical Nurse to provide professional nursing care in patients' homes under RN supervision. Responsibilities include assessments, care planning, documentation, and coordination with the multidisciplinary team. Requires LPN license, BLS, 1 year acute nursing experience, and valid driver’s license. Home health experience preferred.

Qualifications
- High school diploma or equivalent.
- Graduation from an accredited practical nursing program.
- Licensed Practical Nurse (LPN) license.
- One year of acute nursing experience.
- Basic Life Support (BLS) certification.
- State issued driver’s license and personal auto insurance.
- IV certification may be required depending on role.
Responsibilities
- Provide nursing care to patients and families in the home setting.
- Assess patient needs and implement nursing interventions per plan.
- Document data in Medicare-compliant records and progress notes.
- Coordinate with RN Case Manager and multidisciplinary team.
- Follow the established care plan with measurable goals.
- Educate patients and families and promote independence.
- Maintain professional boundaries and ongoing competencies.
- Adhere to safety and regulatory guidelines in home health.
