Qualifications
- Active Massachusetts LPN License (Required).
- Experience in home care, assisted living, or hospital environments is a plus.
- Strong clinical communication and teamwork skills.
- Reliable transportation to attend shifts on time.
Responsibilities
- Provide skilled nursing care and clinical assessments in a home setting.
- Administer medications and treatments according to care plans.
- Assist with complex Activities of Daily Living (ADLs) and mobility support.
- Monitor health status, manage clinical paperwork, and provide medication reminders.
- Maintain clear, reliable communication with clients, families, and care coordinators.
