Job summary

Tufts Medicine Care at Home in Lowell, MA seeks a Home Health Registered Nurse to deliver nursing care in patients' homes, using the nursing process to assess, plan, implement and evaluate. Qualifications

• Active RN license in state of practice; licensure rules must be followed.
• Valid state Driver’s License.
• Minimum 1 year of acute medical/surgical nursing experience.
• Basic Life Support Certification.
• If hired before 9/28/2026, CPR certification required on hire; if after, CPR/BLS due within 30 days.

Responsibilities

• Assess patients' physical and psychosocial needs following Standards of Nursing Practice and Agency policies.
• Coordinate care with patients, families and other health care providers.
• Document patient data accurately in the EMR and maintain timely communication with the care team.
• Provide nursing coverage on a rotating basis for evenings, nights, weekends and holidays.
