Job summary

Tufts Medicine Care at Home in Lowell, MA invites an experienced Home Health Registered Nurse (RN Case Manager) to assess, plan, and deliver patient‑centered care in clients' homes.

The role requires MA RN licensure (and NH licensure in some cases), a valid driver’s license, 1 year of acute medical/surgical nursing, and BLS. BSN preferred with potential for growth within a supportive home health team.

Qualifications

• Massachusetts RN Licensure and NH Licensure.
• Valid state issued Driver's License.
• One (1) year of acute medical/surgical nursing experience.
• Basic Life Support Certification.

Responsibilities

• Assesses patients' physical and psychosocial needs following Standards of Nursing Practice.
• Documents patient data accurately in the electronic patient record.
• Completes physicians' orders and care plans, including OASIS where applicable.
• Coordinates care and discharge planning with the health care team and community resources.
• Provides nursing coverage on a rotating schedule including evenings, nights, weekends and holidays.
• Maintains safety and infection control standards in the home setting.
