Excelin Home Health is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in home health settings under the supervision of a registered nurse.

Responsibilities include implementing the care plan, timely documentation, patient and family education, and coordination with the rest of the care team. The position requires an active LPN/LVN license, CPR certification, and strong communication skills.

Formación

• Graduate of an accredited practical nurse or vocational nursing program.
• Has at least three years nursing experience and one year in community/home health or medical/surgical experience.
• Current license as a LPN/LVN in the state.
• CPR certification.
• Demonstrates good verbal and written communication and organization skills.

Responsabilidades

• Provides direct patient care under supervision of a registered nurse.
• Implements plan of care initiated by the RN.
• Documents accurately and timely per the plan of care.
• Educates patient and family as pertinent to diagnosis and plan of care.
• Coordinates home health services and communicates needs to RN/Clinical Director.
• Uses equipment and supplies efficiently.
• Performs duties as assigned by the RN.
