Pathways Healthcare is seeking a Licensed Practical Nurse - Home Health to join our physician- and nurse practitioner-led team in Spencer, MA. This full-time role offers Monday to Friday hours (8:30–5:00) with a focus on delivering skilled nursing care at patients’ homes under RN supervision.

Ideal candidates have 2+ years of nursing experience (geriatrics preferred), a nursing degree, EMR proficiency, and a valid driver’s license with a reliable vehicle.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• New nurses welcomed; training program available.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse and in accordance with the established plan of care.
• Monitors the patient’s condition and collects relevant health information during each visit, reporting changes in condition or needs to the supervising RN and other care team members.
• Contributes to the development and modification of the plan of care by providing observations and updates.
• Performs preventive and rehabilitative nursing procedures as per plan of care.
• Administers medications and treatments as prescribed and in the plan of care.
• Observes and documents patient responses and reports significant changes.
• Completes and submits accurate clinical documentation about patient’s condition and response to treatment.
• Communicates with supervising RN, physician, and care team regarding patient needs and changes.
• Provides education to patients and caregivers about medications, treatments, and safety.
• Maintains a safe home environment and participates in interdisciplinary team meetings.
