Job summary

Pathways Healthcare is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing home health team in the Boston area, delivering direct patient care in patients' homes while collaborating with RNs, physicians, and families to ensure high-quality outcomes.

The role is per diem with primary office in Westwood, MA and service coverage across Boston neighborhoods such as Jamaica Plain, West Roxbury, Hyde Park, Dorchester and South Boston.

Qualifications

• Massachusetts LPN license required.
• Graduate of an accredited practical nursing program.
• Experience in home health, hospice, or geriatrics preferred; EMR proficiency required.

Responsibilities

• Provide skilled nursing care under supervision of RN per plan of care.
• Monitor vital signs and document patient condition and response to treatment.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on medications and disease management.
• Maintain accurate clinical documentation.
• Coordinate supplies and services for patient care.
• Support quality improvement and compliance standards.
• Accept and fulfill assigned patient visits and schedules.

Pathways Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team. This role provides direct patient care in a home health setting while collaborating closely with RNs, physicians, patients, and families to ensure high-quality care and positive patient outcomes.

Position Details:

• Main Office Location: Westwood, MA
• Service Area: Boston, Jamaica Plain, West Roxbury, Hyde Park, Dorchester, South Boston areas.
• Job Type: Per Diem

Responsibilities:

• Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
• Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
• Administer medications and treatments as prescribed.
• Assist with implementation and ongoing evaluation of patient care plans.
• Educate patients and caregivers on medications, treatments, and disease management.
• Communicate changes in patient condition to the supervising RN and physician as appropriate.
• Maintain accurate and timely clinical documentation.
• Promote a safe and comfortable environment for patients.
• Coordinate necessary supplies, equipment, and services for patient care.
• Support quality improvement initiatives and compliance standards.
• Accept and fulfill assigned patient visits and schedules reliably.

Qualifications:

• Current and active LPN license in Massachusetts.
• Graduate of an accredited practical nursing program.
• Previous nursing experience preferred, especially in home health, hospice, or geriatrics preferred.
• Proficient with electronic medical records (EMR).
• Excellent communication and interpersonal skills.
• Current BLS CPR certification.
• Valid driver’s license and reliable insured vehicle required.
