Job summary

Qualifications

• Active LPN license in the state of practice.
• Valid driver's license and vehicle insurance, or reliable transportation.
• CPR certification preferred (within 90 days of hire).
• Excellent communication and organizational skills.
• Ability to manage multiple tasks and work independently.

Responsibilities

• Provides high quality clinical services within the scope of practice, per plan of care and infection control procedures.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Coordinates care, reports patient needs, and communicates with patient, family, physician, and care team.
• Adapts to care in varied home environments regardless of client demographics.
