Job summary

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle access or reliable transportation.
• CPR certification preferred or ability to obtain within 90 days.

Responsibilities

• Provides high quality clinical services within scope of practice per plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, reports patient needs and changes in condition, communicates with patient, family, physician and team.
• Function effectively in diverse home settings and with varied patient backgrounds.
