Job summary

Home Sweet Home seeks a Licensed Practical Nurse (LPN) to deliver hands-on skilled nursing care to patients in their homes across Central and South Phoenix, under RN supervision, following individualized care plans and documenting visits in the EHR.

Ideal candidates hold a current Arizona LPN license, have at least 1 year of nursing experience (home health preferred), and can travel to patient homes in the Maricopa area. Schedule: M-F 8-5; Pay-Per-Visit: $45–$55 USD.

Qualifications

Arizona LPN license in good standing.
Graduate of an accredited vocational nursing program.
Minimum 1 year of nursing experience (home health preferred).
Ability to travel to patient homes in Central Phoenix and surrounding Maricopa areas.
Strong communication and documentation skills.
Proficiency with electronic charting and care coordination software.

Responsibilities

Provide skilled, patient-centered nursing care based on individualized care plans.
Accurately document all visits, assessments, and treatments using electronic health records (EHR).
