Job summary

UnitedHealth Group is hiring a Licensed Practical Nurse (LPN) for home health services. The LPN will deliver high quality patient care in patients' homes, document visits in the electronic medical record, and participate in care coordination with the care team.

A valid LPN license in the practicing state and CPR certification are required, with flexibility to meet client needs. The role involves working with diverse patients in home settings, maintaining infection control, and following the plan

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, communicates patient needs and changes in condition with the team.
• Functions in diverse home environments and with patients of varying ages, races, and backgrounds.
