Job summary

Dartmouth Health Home Care is seeking a Licensed Practical Nurse to join our home health and hospice team in Springfield, VT and surrounding areas. You will perform patient-directed nursing tasks under RN supervision and collaborate with an interdisciplinary team to support patients in their homes.

The role offers full-time, part-time, and per diem opportunities with day shifts. A new hire retention bonus of $10,000 is available under certain conditions, and compensation reflects the Vermont

Qualifications

• Must hold at least one active LPN license in VT or NH at time of hire
• Graduate of an accredited school as a Licensed Practical Nurse
• One year of experience as an LPN in a home health care setting is preferred

Responsibilities

• Provide prescribed nursing care to individuals and families in their residence under RN supervision
• Participate in patient case conferences as part of the interdisciplinary team
• Assist the RN in developing and modifying the plan of care as needed
• Observe, document, and report patient needs and responses to care
