Job summary

Dartmouth Health Home Care is seeking a Licensed Practical Nurse to deliver professional nursing care in patients' homes within Springfield, VT and surrounding areas. You will work under RN supervision within an interdisciplinary team, ensuring high-quality home health and hospice services.

The role requires VT/NH LPN licensure, a valid driver's license and vehicle insurance, with a focus on patient-centered care and community health impact. A New Hire Retention bonus may apply.

Qualifications

• Active LPN license in VT or NH at time of hire.
• One year home health care experience preferred.
• Graduate of an accredited LPN program.

Responsibilities

• Provide nursing services to individuals and families in their residence under RN supervision.
• Perform prescribed individualized patient care as delegated.
• Participate in patient case conferences as appropriate; be an active member of the interdisciplinary team.
• Assist the RN in the preparation, evaluation and modification of the patient's plan of care.
• Observe, document and report to the RN patient needs and reactions to care.

Responsibilities

The Licensed Practical Nurse:

Under the supervision of a RN and in accordance with agency policies and state practice, the Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.

• Performs prescribed individualized patient care to patients as delegated.
• Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
• Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
• Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Qualifications

• Must hold at least one active Licensed Practical Nurse license in either VT or NH at the time of hire. Employees must have active Licensed Practical Nurse licenses in both VT and NH within 30 days of their hire date.
• Graduate of an accredited school as a Licensed Practical Nurse
• One year of experience as an LPN in a home health care setting is preferred

Required Licensure/Certifications

• Must hold at least one activeLicensed Practical Nurse license in either VT or NH at the time of hire. Employees must have active Licensed Practical Nurse licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license, and a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
